About the Role:

Here’s what you can look forward to as a Care Coordinator with us:

  • Working with your clients, family members, and their supports to identify their needs and ensure that the support provided meets them.
  • Support from your Manager as well as your team members to achieve goals.
  • Updating Care Plans and Budgets for our clients to ensure that their care needs and deterioration of health are well documented.
  • Working with our large list of like-minded suppliers to set up the ideal care and support services for your clients.
  • A warm and friendly work environment at one of the largest Home Care Package Providers in Australia.

 

About you:

We value a positive and enthusiastic attitude and will train the right person.

Here’s the type of person we are looking for:

  • An enthusiastic individual who understands the importance of helping people and is passionate about providing excellent customer service.
  • A caring and professional nature.
  • Be fluent in English and Mandarin
  • Ideally able to also speak Cantonese
  • Relevant aged care/health qualifications (For example Aged Care, Disability, Social Work, and/ or Community Services qualifications)

Desirable:

  • Care Management experience.
  • An understanding of Home Care Packages and the Aged Care Quality Standards
  • Experience with updating/ reviewing Care Plans

Although this is not a nursing position, the position would be suitable for an enrolled nurse, someone with an allied health background, or someone with experience in residential and/or community aged care.
