About the role.
This flexible part-time role reporting to the Clinical Team Leader is vital to the continued success of an organisation that is dedicated to quality and integrity, and provides an inclusive working environment. Based in the Community you will play a pivotal role in delivering excellent customer service through listening to clients and their family’s needs while providing appropriate solutions.
Key Accountabilities include:
- Provide clinical nursing service to clients through assessments, reviews and delivery of client’s individual care plans.
- Knowledge and understanding of government funding related to Home Care Packages, Department of Veteran Affairs, NDIS, etc.
- Develop focused care plans around identified care domains e.g. Wound Care, Medication Management, Chronic Disease Management, Incontinence Management, Catheter Care.
- Support clients and their carers/families living with Dementia and End of Life Care/Palliative Care.
What you will bring.
- A great attitude with the willingness to learn and adapt as we work in a constantly changing world.
- You have outstanding time management skills with the ability to communicate effectively across different groups of workers and clients.
- You have great work ethics and will confidently contribute to improve processes and suggest new ideas.
- Skills in clinical nursing practice with an understanding of contemporary nursing theories.
- Problem-solving abilities with the ability to think critically.
- Strong interpersonal and verbal / written communication skills.
- Ability to treat client information in strict confidence at all times and work in a fast-paced environment.
- You have at least 2 years experience as a Registered Nurse, preferably in a community setting.
Essentials:
- Current AHPRA registration with minimum two years nursing (RN) experience, preferably in a community setting
- A current driver’s licence and access to a reliable car with comprehensive insurance
- Smart phone, internet and email access
- Current National Police Check and Working With Children Check or willingness to obtain
- Current First Aid Certificate
- Flu vaccination
- Work rights in Australia
